Hospitals give out the shortest prescription amounts they can to keep their prescribing rates down.
Gp's are constantly audited by the nhs who publish prescribing results costs by practice and push cheaper generic prescribing.

if you ask your gp for a blood test...they will send you to various blood clinics...your gp gets paid for the blood test...but another clinic does the actual test...the nhs has to pay for both services...its madness...some gp surgeries have sacked many staff and are now just money making setups...i dont want to shame gps but some local surgeries have been taken over by foreign money makers and have sacked most of their staff and just rake in the money...remember if your gp refers you for a cancer investigation it costs them a lot of money for their practise....so you must kick up a fuss if they wont refer you!...its time to close surgeries and put all services back into the hospitals and nhs....your life may depend on it

Every Health Authority has a local Clinical Commissioning Group controlling the budget of services in their area.

GP's are contracted to do blood tests.
GP's are not allowed to order some blood tests because expensive ones are rationed/discouraged unless clinically indicated.
If the G.p does not do the test the cost is on the tab of other nhs provision funded by the CCG.
Regarding cancer treatment.If anything Gp's are over cautious and will refer to a hospital based consultant (as per guidelines) any patient they
suspect may have cancer.
Any Dr missing a diagnosis could face suspension and summons before
Manchester disciplinary panel.
Tests do not cost the G.p anything.

I know of a gp in Southport who has misdiagnosed two people who had cancer and she's still practising gp.

Then my blood tests to monitor me , despite a letter from the hospital the receptionist argued over giving me an appt because she could not find the letter on her system , the letter turned up after a search .
Amazing how the words "Cancer op" and "follow up tests" also eluded her ! , I was not asking for a blood test for the fun of it ..
I went throu the covid bloods taking routine and then my scan appts did not turn up as expected ... because the blood results had not turned up at the hospital to trigger the next test stage ! , despite a reminder to the surgery receptionist to post them I still waited , follow up blood tests are also not done despite a discussion with the Dr ... surly this is a routine task and should not be open to mistakes like this as its just an entry on the computor system to make a booking .
Last weeks appt in the hospital with the consultant , I get to see the blood test result is missing from the system , last one was in Feb when the op took place , I now have an emergency scan booked , and more bloods were taken . It will be interesting to see the results
No one seems to have ownership of a persons case for following up on the welfare of the patient , for all they knew I could of gone home and dropped dead on the floor , I have had to chase every single bit of care to make sure it was done .
It seems to be a geographical lottery as to whether you receive the attention you need or not. All GP's surgeries should be moving back to pre-Covid services according to:

Some people in the South are receiving phone calls from their Doctor's surgeries, checking up on their health if they have underlying conditions or if they have recently had operations etc., But locally it would appear that patients are being transferred from on place to the next in a circle - you can forget the 111 calls too.

the out of service 7day gp is a private firm making a lot of money, the directors that own the company, are actually gps, the very same gps that refuse to see you at the surgery but get paid by the private company who in turn charges the nhs....in effect they get paid twice for your visit..without actually seeing you...as i said the nhs is a money making sham...look at out two largest surgeries in southport now private companies who have shed most staff and evade seeing you

Summary
1 Prior to 2004 GPs were working to a 90 year old contract clause which made them personally responsible for providing round the clock services
2 For many reasons by the turn of the century this had become unsustainable
3 The Department of Health also had an agenda to integrate out of hours services
4 The 2004 passed the responsibility for organising out of hours GP services from individual GPs to Primary Care Trusts
5 The amount of money given up by GPs equated to what they were paid for out of hours work prior to 2004
7 Full -Time GPs continue to work hours above the national average but there has been an increase in the number of GPs working part time
8 Any perceived inadequacies in the current Out of Hours GP services are the responsibility of the Primary Care Trusts not of GPs
