Apart from the fact that Everton Mints/Black and White mints have not been banned, nobody has banned Midget Gems, M&S have renamed them because language changes, brands change all the time, the branding was wrong. It's nothing new, you can still buy the bloody things.

Jif - Cif - nobody in the non-English world can say "Jif"
Kentucky Fried Chicken - KFC because of civil action by state of kentucky

I knew about 'Scope' and 'Cif', I didn't know about KFC. I thought they'd just shortened it because they tend to dumb things down / shorten brand names.
I don't know if e-cigs are sold in terms of 'puffs', but I know they still sell Cheesy Puffs and puff pastry.
Mainly because it isn't 1973.
As for offensive, I suppose it's how word / phrases are delivered and how they affect a particular group. We're happy to be called 'Brits', but the shortened word for someone from Pakistan is offensive. Because it was used as a slur rather than a simple geographical identifier.
Language evolves. Tell your kids something is 'fab', or say you've got no 'bread' meaning money, and they'll look at you like you've got two heads.
